I offer assessment and documentation support for individuals seeking Leave of Absence (LOA) or Family and Medical Leave Act (FMLA) paperwork related to mental health concerns.
This service includes a comprehensive clinical assessment to evaluate symptom severity, functional impairment, and diagnostic criteria. If symptoms meet the criteria for a clinically impairing mental health condition, I can complete the required LOA or FMLA documentation.
Following the assessment, a personalized treatment plan is developed to support symptom reduction and functional improvement. Treatment typically includes weekly individual therapy sessions focused on evidence-based strategies to decrease symptom severity, improve coping skills, and support a successful return to work when appropriate.
