1. Our Legal Duties
Prosper Integrative Health is required by law to:

2. How We May Use and Disclose Your Health Information
We may use or disclose your PHI without your written authorization for the following purposes:

To provide, coordinate, or manage your health care, including sharing information with other providers involved in your care.
Payment
To bill and receive payment from health plans or other payers.
Health Care Operations
For practice operations such as quality assessment, staff training, licensing, credentialing, audits, and compliance reviews.
As Required by Law
Including disclosures required by federal or Texas law, court orders, subpoenas, or regulatory agencies such as BHEC.
Public Health and Safety
When necessary to prevent a serious threat to health or safety, or as required for reporting abuse, neglect, or exploitation.
3. Uses and Disclosures Requiring Your Authorization
We will obtain your written authorization before using or disclosing your PHI for:
Marketing purposes
Sale of PHI
Most uses of psychotherapy notes (if applicable)
Any other purpose not described in this notice
You may revoke an authorization in writing at any time.
4. Your Rights Under HIPAA and Texas Law
You have the right to:
Access Your Records
Request to inspect or obtain copies of your health records.
Request Corrections
Ask us to amend your records if you believe information is incorrect or incomplete.
Request Confidential Communications
Ask us to contact you in a specific way or at a specific location.
Request Restrictions
Ask us to limit how we use or disclose your PHI (we are not required to agree in all cases).
Receive an Accounting of Disclosures
Request a list of certain disclosures we have made.
Receive a Paper Copy of This Notice
Even if you have agreed to receive it electronically.
5. Texas-Specific Privacy Rights (HB 300)
Under Texas law, you have additional rights, including:
Protection of your PHI beyond HIPAA minimum standards
The right to know how your information is used and disclosed
The right to file complaints regarding misuse of your PHI
Limits on electronic disclosures and re-disclosures of PHI
Prosper Integrative Health trains all workforce members on Texas privacy laws as required.

8. Changes to This Privacy Policy
We reserve the right to change this Privacy Policy at any time. Changes will apply to all PHI we maintain and will be posted on our website and available in our office.

1. How to Request Your Health Care Records
You have the right to request access to or copies of your health care records maintained by Prosper Integrative Health.

Requests will be processed in accordance with applicable state and federal law.
2. How to Contact the Council
If you have questions about your rights under Texas behavioral health laws or want to contact the Council for information:
