Please reach us at Jada@prosperintegrativehealth.com if you cannot find an answer to your question.
First, we need to know if we will be a good fit! Schedule a free 15 minute phone consultation online, by phone (940-312-1031) or email.
If you’re ready to schedule an appointment, please visit my secure booking portal and create an account. You’ll be asked to enter your personal information, provide insurance details if you plan to use insurance, and complete the required intake documents. Completing these steps in advance helps ensure a smooth start to services and allows us to make the most of your first session.
During your intake session, we will spend time getting to know you, including your personal history, past medical and mental health care, and what you hope to achieve in therapy.
Toward the end of the appointment, we will talk about a diagnosis (if applicable) and next steps, which may include ongoing therapy or a referral to another provider.
The first appointment is a consultation to determine if we’re a good fit and if ongoing treatment would be helpful. It does not guarantee that treatment will continue.

I provide clinical assessment and documentation support for individuals seeking LOA or Family and Medical Leave Act (FMLA) paperwork related to mental health concerns.
This service includes a comprehensive clinical assessment to evaluate symptom severity, functional impairment, and whether diagnostic criteria for a clinically impairing mental health condition are met. The assessment focuses on how symptoms impact occupational functioning.
No. Completion of LOA, FMLA, or accommodation paperwork cannot be guaranteed prior to or following assessment. All documentation is based on clinical findings, diagnostic criteria, functional impairment, and professional judgment.
Clients are responsible for obtaining all required forms from their employer, human resources department, school, or academic institution and providing them for review and completion. I am unable to obtain forms on a client’s behalf.
Yes. Clients are responsible for inquiring with their employer or benefits administrator regarding available leave options, including LOA, FMLA, and/or short-term disability benefits. I do not determine benefit eligibility or approval.
Approval of LOA, FMLA, accommodations, or short-term disability is determined solely by the employer, insurance provider, or academic institution. Completion of clinical documentation does not guarantee approval of leave or accommodations.
If clinical criteria are met, I may complete applicable documentation. Following the assessment, an individualized treatment plan is developed to address symptom reduction and functional improvement. Treatment often includes weekly individual therapy sessions utilizing evidence-based interventions to support coping, symptom management, and return to work or school when clinically appropriate.
Yes. I provide assessment and documentation support for workplace and academic accommodations related to mental health conditions when clinically appropriate.
Accommodation recommendations are based on clinical findings, identified functional limitations, and professional judgment. When criteria are met, I may provide documentation recommending reasonable accommodations to support performance, access, and well-being. Recommendations are individualized and not guaranteed.
